An Ohio nurse has been indicted in connection with the death of an 84-year-old woman at a Cuyahoga County nursing facility.
Amber Henderson faces multiple charges following the tragic death of Alvera Meuri at the Avenue at Warrensville Care and Rehabilitation Center. This case is about the massive failure of both the individual healthcare provider and the facility to protect their residents.
Incidents like these are common in many nursing home abuse cases.
Here’s what we know…
A Preventable Tragedy
The incident happened on December 23, 2024, at the Avenue at Warrensville Care and Rehabilitation Center in Cuyahoga County. Henderson began her shift at 9:15 p.m. Her patient list included 84-year-old Alvera Meuri.
The nurse from the previous shift told her that Alvera could not walk on her own. The last check-in for her was at 8:40 p.m.
However, the older woman was gone when Henderson checked Alvera’s room. Instead of immediately alerting staff or following missing resident protocols, Henderson believed Alvera had left with family. She continued to work.
After a few hours had passed, Henderson finally tried to call Alvera’s family at 12:35 a.m., but no one answered. At this stage, no action was taken to locate the missing woman.
Finally, at 6:30 a.m., which was almost 10 hours after Alvera was last seen, Henderson issued a “Code Purple” for a missing resident emergency. Once the police arrived at 8:00 a.m., nurses discovered Alvera’s body. She was found lying on her back on the facility’s patio. The resident had spent the entire night in the freezing cold. She was rushed to a nearby hospital but did not survive.
The Cuyahoga County Medical Examiner stated that hypothermia caused her death.
A System That Failed
This is an example of a death that never should’ve happened. The investigation also uncovered several troubling details regarding Henderson’s actions and the facility’s procedures.
According to a report from CBS affiliate WOIO-19, it was discovered that all new nurses at the Avenue at Warrensville undergo four weeks of training.
During this time, the nurses learn about specific protocols on what to do if a resident goes missing. Along with that, staff members are also required to check on residents every two hours. None of these protocols were followed in Alvera’s case.
However, that was not the full extent of the negligence. Henderson also reportedly falsified records. She would claim to complete certain patient care tasks that she never actually did.
Another issue was the facility’s security failure. Every exterior door was to be locked and accessible only by a keypad system.
Unfortunately, there was an unlocked door near Alvera’s room. That door led to a staircase that led to the patio, where she became trapped. Since the doors were locked from the outside, Alvera had no way to re-enter the building.
Due to her role in the death of Alvera Meuri, Henderson was indicted on:
- One count of involuntary manslaughter
- One count of tampering with records
- One count of patient neglect
Henderson is set to be arraigned at a later date. If convicted, she could serve several years in prison for involuntary manslaughter and patient neglect.
The facility may face state and federal investigations due to non-compliance with safety standards and patient care regulations. If found negligent, the Avenue at Warrensville Care and Rehabilitation Center could be subject to fines, lawsuits, and potential revocation of its operating license.

This incident was not just a nursing mistake. It was a catastrophic failure of care, protocol, and basic human decency.
For Alvera Meuri’s family, no criminal conviction will bring back their loved one or erase the trauma of knowing she spent her final hours in the freezing cold.
